Breast augmentation with breast implants with a round profile (= colloquially known as round implants) The 23-year-old patient presented with a request for breast augmentation. She wanted a round implant profile to emphasise the cleavage. After a detailed examination and consultation, the operation was performed with implants from Motiva® using the subfascial technique. The result corresponds to the pre-operatively discussed goal. Detailed information on the case 👇 Correction of a malpositioned implant (Bottoming-out) This case shows the plastic surgery correction after an external breast augmentation. Due to weak connective tissue and the high weight of the implants (450 cc), the implants sank far below the natural underbust fold. This resulted in a pronounced „bottoming-out“. The implants dislocated approx. 2-3 cm caudally (below) on both sides. This led to a considerable ptosis (sagging breast), thinning of the soft tissue mantle in the upper pole and painful malpositioning. The downward shift of the centre of gravity created a considerable leverage effect on the spine, which led to chronic back and neck pain as well as postural deformities in the patient. Our surgical solution for reconstruction: Implant change: replacement with significantly lighter (B-Lite®️) and smaller implants (300 cc) to reduce the static pressure on the supporting apparatus and the spine in the long term. Mastopexy: Tightening of the excess skin mantle and anatomical repositioning of the nipple-areola complex. Internal stabilisation: Reconstruction of the underbust fold and use of a supporting mesh (GalaFlex®️) to prevent further sagging in the sense of an „inner bra“. This made it possible to restore a stable, anatomically correct breast shape and eliminate the physical discomfort caused by the malformation. This is the presentation of a medically indicated revision operation to correct functional complaints (incorrect posture/pain syndrome).
