The International Society of Aesthetic Plastic Surgery (ISAPS) is a worldwide organisation for plastic surgeons specialising in aesthetic and cosmetic procedures.

ISAPS promotes professional exchange, continuous education and international co-operation among surgeons. National representatives, known as National Secretaries, support the members in their respective countries by passing on information and organising events.

In Germany, Prof Dr Noah has assumed this honorary role of National Secretary since 2018.

A strong increase was reported for non-surgical skin tightening (+38.9 %) and chemical peels (+33.3 %).

Demographic distribution

Most breast augmentation (54 %), rhinoplasty (60.1 %) and external genital surgery (48.4 %) were performed on people aged 18-34.
Botulinum toxin injections were most popular in the 35-50 age group (47 %).

Regional focus

The USA led the global rankings with over 6.1 million procedures, followed by Brazil (3.1 million) and Japan (1.63 million).
Brazil recorded the highest number of surgical procedures worldwide, while the USA is the leader in non-surgical procedures.

Medical tourism

A significant proportion of aesthetic procedures are carried out by international patients. Countries such as Colombia, Turkey and the United Arab Emirates (UAE) are popular destinations for medical tourism, with up to 45 % of patients coming from abroad (e.g. in Tunisia).

What is a mini-lift?

At what age does a facelift make sense: at 40, 50, 60? "When the perceived age no longer matches the reflection in the mirror and minimally invasive methods are no longer effective, is the right time for a facelift," says Prof. Dr. E. Magnus Noah.
The facelift is the supreme discipline of aesthetic plastic surgery and its results are unrivalled. No other procedure is able to correct the signs of ageing in the face so effectively. "State-of-the art is the SMAS facelift," explains facelift expert Noah. With this procedure, not only the skin is tightened, but also the underlying deeper tissue of muscles, fat and connective tissue.

The so-called SMAS (Superficial Muscular Aponeurotic System) plays the main role in the complex ageing processes of the face: if its elasticity decreases, the face sags downwards and loses its youthful contour.

In the face, there are "fixed" and "mobile" SMAS parts, but the ageing process mainly takes place in the mobile parts. For this reason, he only prepares and tightens the tissue in these mobile areas while maintaining the classic incision. "Although this technique is more demanding to operate and requires very good anatomical knowledge, it is more comfortable for the patient as the healing process is faster. The results are even more stable than if you were to prepare the entire SMAS," says Prof. Noah, praising the advantages of the Deep Plane technique.
In order to reverse the ageing process in the long term, the face is rejuvenated in its entirety. The plastic surgeon reduces the mobile SMAS areas as well as the fatty tissue with sure instinct and thus restores the youthful contours. The result is a natural and unoperated appearance. The face looks the same as it did about ten years ago.
"Of course, the ageing process continues after the lift," says Noah, "but the years gained remain for life." So if a patient looks ten years younger after a facelift, she will look that much younger in 15 years.
We have been carrying out the deep plane facelift since 2016
The decisive breakthrough in the development of the method came with the work of Dr Brian Mendelson. His anatomical studies showed that certain avascular (vessel-free) spaces exist under the SMAS. These allow particularly safe and effective preparation at depth. The so-called retaining ligaments - holding structures in the face - can be specifically released and the tissue repositioned in its natural direction.
Based on these findings, we have been performing the Deep Plane Facelift in its most modern form at the Noah Clinic since 2016. Several hundred successful procedures confirm the effectiveness of this method.

Slight swelling and bruising around the mouth after the injection is normal and nothing to worry about. Cool the mouth regularly and allow yourself some rest on the day of treatment.
Also avoid direct sunlight or solarium for about a week.
Patients are often fit for socialising the next day, but the swelling can last up to three days in rare cases.

When does the effect begin?

Lip-Flip takes effect within 3-5 days.

How long does the result last?

The result lasts about 3-5 months. If necessary, the treatment can be repeated and refreshed.

If fat cells are exposed to cold for a longer period of time, they die. This scientific finding led to the development of a non-invasive body sculpting treatment: cryolipolysis or better known as "Coolsculpting". The procedure is certified by the American Food and Drug Administration (FDA) for the reduction of fat cells through exposure to cold and has been proven to be effective. "For the removal of small fat deposits, this new procedure is a sensible alternative to classic liposuction," explains Prof. Noah, who performs this innovative method in his clinic.

Often, it is only small pads on the thighs, knees, arms or belly that cloud the body image. In men, hip rings or a small belly are the most frequent reasons for going to the plastic surgeon. The good news is that these fat deposits can be permanently removed safely and painlessly with cryolipolysis. The local treatment takes about one hour: "The fat cells are destroyed by the cold treatment and then metabolised by the body in the following weeks," Prof. Noah explains the procedure. Therefore, it takes two to three months until the final result is visible. If necessary, further treatments can be performed on the treated body region to optimise the result. However, if larger amounts of fat need to be removed, liposuction is still the method of choice, the expert clarifies.

A young face is characterised by its heart shape, which resembles a V, with plenty of volume in the cheek area. With age, however, the shaping fat compartments in the midface sag and the face loses its youthful V-shape. It becomes rounder and more A-shaped. Incidentally, this change in the shape of the face means that a person's age can be seen from a distance, even if wrinkles are not visible from a distance.

The good news is: Today, you no longer have to put up with that. This lost volume can be compensated with a treatment with Own fat or Hyaluron and give the face back its rejuvenating volume," explains Prof. Noah. The focus here is on the cheek area, which shrinks with the years and makes the face look older. Filling the fat area is therefore the logical consequence for an optical rejuvenation effect.

Plastic surgeons are increasingly using the body's own fatty tissue for volume treatment - thanks to technical developments and new findings in stem cell research. This is because fatty tissue contains a lot of stem cells; these are essential for the body to accept the injected fat. The advantage over other fillers is that the patient's own fat integrates naturally into the tissue. "The required fat cells are removed with a cannula from where they are dispensable - for example, from the hips or abdomen," explains Prof. Noah. They are then prepared and injected into the face. "Overcorrections are hardly possible, because the body breaks down excess fat cells again," knows the specialist, who has been working with autologous fat injections for years.

Alternatively, a volume lift can also be performed with hyaluron. Especially for a volume treatment, preparations are available that are characterised by their special viscosity. "This makes them easy to inject into the deep layers of the skin and ensures a natural result," explains Prof. Noah. To give the cheeks back their youthful fullness, Noah treats precisely defined points in the region, on the cheekbone and below the lower lid. A welcome side effect: the unloved nasolabial folds are automatically smoothed out by the lifting effect of the volume treatment, as these are usually caused by the sagging cheek fat.

When the sweat is pouring - help from a medical specialist

When temperatures rise, the body reacts by sweating to cool itself down. This is a natural and sensible process. Some people, however, sweat excessively all year round; it literally drips from the armpits and hands and huge sweat patches form on blazers and jackets that cannot be hidden. This often has serious consequences for those affected: They feel insecure and inhibited, because sweat is usually equated with a lack of personal hygiene.

So-called hyperhidrosis affects 1 to 2 percent of people in Germany. "This excessive sweating can be treated surgically by surgically removing the sweat glands in the armpit," explains Prof. Magnus Noah, who has been relieving those affected of their suffering, which is so embarrassing for them, for many years.

Under local anaesthetic, a small incision is made in the armpit and a specially designed suction cannula is inserted into the zone of sweat gland accumulation. The sweat glands are softened with tumescent fluid and then removed. Only the affected area below the skin is treated in order to spare the tissue and nerves in the armpit. The procedure only takes half an hour. The result: In 80 percent of the patients treated, sweating is significantly reduced after this operation.

Alternatively, the armpits can also be temporarily "dried out" with botulinum, the active ingredient known from wrinkle treatment. This method is also suitable if palms or feet are affected by hyperhidrosis. The active ingredient acts on the nerves that are responsible for sweating. This effectively stops the formation of sweat. Since the palms of the hands and soles of the feet are very sensitive to pain, Prof. Noah performs the injections under local anaesthesia to make the treatment as comfortable as possible for the patient. The effect takes effect after three to six days and lasts four to six months. After that, the active ingredient has broken down and follow-up treatment is needed. "Some patients are treated with botulinum especially in the summer months against their excessive perspiration in order to be able to enjoy the most beautiful season in a relaxed way," Prof. Noah explains the motivation of his patients.

The nicer the weather and the higher the temperatures, the shorter the sleeves. At the same time, of course, the arms should look particularly beautiful and toned. Women in particular have attached great importance to this, and not just since Michelle Obama, as they like to look flawless in strapless tops. Unfortunately, this is not always the case, because with a few kilos too many or with increasing age, unattractive bulges appear on the upper arms, which can be very disturbing for those affected. Also, the firmness often leaves a lot to be desired. "Simply removing the fat pads is usually not enough," Prof. Noah reports. "In these cases, a Tightening of the tissue for a beautiful and appealing result to be achieved."

For this, Prof. Noah uses the BodyTite™ procedure. This procedure combines the classic Liposuction with a simultaneous radiofrequency treatment that immediately tightens the tissue. Two in one, so to speak.

After the arms have been anaesthetised with a tumescent local anaesthetic and the tissue has been softened, a cannula fitted with an electrode is inserted into the fat layer just under the skin. Its radio waves generate heat in the fat and subcutaneous tissue, which tightens the connective tissue and simultaneously liquefies the fat, which is then suctioned out via the cannula. Afterwards, the patient has to wear arm corsets for about four weeks so that the skin is tightened. Bruising can discolour the area for about two weeks. The reward for the effort: not only have the unloved fat pads disappeared, but the entire tissue has been tightened and rejuvenated.

Congenital drooping eyelids can look cute when you're young - like Jennifer Lawrence. As we age, many other people develop drooping eyelids, which can make us look tired and grumpy, even when we are in the best of moods.

Because the eye area is the first place where the signs of time become visible. Large, open eyes, on the other hand, stand for youthfulness. If the drooping eyelids are perceived as very disturbing, a surgical intervention can provide relief. "Basically, you have to distinguish between congenital and age-related drooping eyelids," explains Prof. Dr. Ernst Magnus Noah. "Because the procedure usually differs. Therefore, a thorough anamnesis in advance is crucial for an aesthetic treatment result," explains the plastic surgeon.

Age-related drooping eyelids develop mainly due to a sagging of the forehead and eyebrow area. This is because over the years, the muscle, connective tissue and skin layers lose their dynamism and sink downwards. As a result, the forehead presses down on the eye area - drooping eyelids develop.

"In this case, a forehead lift or even an inner eyebrow lift should be performed for an aesthetic result," says Prof. Noah. Because in such cases, merely performing an eyelid correction would not lead to the desired aesthetic result. In a forehead lift, the eyebrows are raised back to their original, youthful position and thus the eyelid area is tightened. The entire skin, muscle and connective tissue layer of the forehead is surgically repositioned. The incision is preferably made at the hairline, using a so-called hairline cut, so as not to alter the natural hairline upwards.

In the case of internal lifting, the eyebrow is moved upwards via the access of the eyelid surgery and fixed (browpexy) In the case of drooping eyelids, which are caused by a pure excess of skin, eyelid surgery is often sufficient to open up the gaze and make the eyes appear larger and more radiant. This is a relatively small operation that is performed under local anaesthetic in only about an hour. The plastic surgeon removes the excess skin and, if necessary, tightens the muscle on the upper eyelid. The suture at the eyelid crease heals quickly and inconspicuously. A tightening of the upper eyelids is often the first surgical procedure patients undergo.

The results are impressive: "Thanks to the now open, large eyes, the patient has a more dynamic overall expression and looks significantly younger," says Prof Noah.

Renée Zellweger's new face and comments on it went around the world in the media. She herself justifies the new, greatly changed appearance of her face with her happiness in private - there is no mention of surgery. However, it is not unlikely that surgery has taken place. Renée Zellweger has not told the untruth, she has merely exercised her right not to say everything and share it with the public. That she is now happy may be believed without a doubt, because according to a recent study, cosmetic surgery does indeed make people happy.

A psychological study conducted by the Ruhr University Bochum (RUB) last year only recently confirmed the long-term positive effect of cosmetic surgery. After their study, the psychologists from Bochum and Basel came to the conclusion that people who had undergone cosmetic surgery showed more joy in life, satisfaction and self-worth after the operation. Since the physical appearance had been surgically changed and now corresponded more closely to the subjective image of beauty, the people concerned showed relief. They also felt healthier and less anxious, according to the study.

The researchers proceeded as follows in the study: 544 patients who had undergone surgery for the first time were compared with two other groups. These were 264 people who had previously wanted cosmetic surgery but had decided against it for personal reasons and around 1000 people from the general population who were not interested in cosmetic surgery. There were no significant differences in psychology or health between the three groups studied.

Prof. Noah can confirm this positive effect after cosmetic surgery, because he has made the same observation in his many years of practice. "For me as an aesthetic plastic surgeon, it is about contributing to the well-being of my patients. And well-being is decisively related to the outer appearance. Very often I experience that my patients are much more self-confident and generally happier after a cosmetic operation thanks to their new appearance."

An important focus in the largest and most important professional association of aesthetic plastic surgeons in Germany is the further training of the members and the collegial exchange of knowledge among each other. "The well-being of the patient and the desire to offer him or her the best possible individual treatment should always come first," demands Prof. Noah.

The VDÄPC is the working and interest group of aesthetic plastic surgeons with a focus on aesthetic surgery. Aesthetic surgery is understood to be those interventions that deal with the restoration and improvement of body shape and visibly disturbed body functions. It seeks to correct the consequences of congenital anomalies as well as acquired or age-related changes in external appearance. For ordinary membership, the VDÄPC requires a specialist in plastic and aesthetic surgery acquired in Germany in the currently valid version of the further training regulations.

Everyone wants to look as good and young as they feel - all the better if they don't have to go under the knife. So-called minimally invasive facial procedures with fillers and botulinum are booming and have been growing for years. "Especially before the holidays at the turn of the year, these quick beautifiers are more in demand than ever," Prof. Noah knows from his practice.

"But here, too, caution and a sense of proportion are important, because every face ages individually and therefore there should be no standard solutions for these measures," explains Prof. Noah. That is why a detailed medical history and consultation are essential for the success of the therapy. In this process, it is analysed exactly what the patient finds disturbing in his or her face: Is it the frown line on the forehead or a deep nasolabial fold that makes him look grumpy? Or is it a washed-out chin line due to age-related loss of contour?

Often it is individual wrinkles that are perceived as unattractive, but increasingly volume loss is also coming to the forefront of treatment requests. Patients often come to me and say, "I just want to look a bit fresher and more rested again," says Prof. Noah. Different treatments have proven successful for the different indications. Usually a combination is the key to success.

Treatment with botulinum has proven to be effective against expression lines such as frown lines, horizontal wrinkles caused by the constant lifting of the forehead, or the unloved crow's feet around the eyes. The proven medical active ingredient is injected directly into the muscle, which is then immobilised. The beautiful result: the skin becomes noticeably smoother. This effect lasts four to six months, after which follow-up treatment is necessary.
If - mostly younger - patients are bothered by individual wrinkles such as the nasolabial fold, these can be effectively injected and smoothed with the latest generation of hyaluron fillers.

For some time now, everyone has been talking about the so-called volume building of the face: with the years, the face loses its contour. The volume in the midface, which gives the face its youthful V-shape, diminishes. This is usually clearly visible in an unclear chin line. This lost volume can be restored with a volume treatment." Here, the individual wrinkle is not treated, but rather the sunken cheek areas are filled up again over a large area with highly cross-linked hyaluron. The face appears fuller and therefore more youthful again," explains Prof. Noah. Especially with volume treatment, less is usually more to achieve a natural result.

After the most beautiful season of the year, summer, many people look sceptically in the mirror these days: Do the bikini and the swimsuit really fit? And how do I look in them? Because the figure is not always what one imagines or dreams of. Especially nasty: even with iron discipline, there are so-called resistant fat pads that just won't disappear even with diet and lots of exercise. For women, these are often the unloved "riding trousers" and for men the euphemistically named "love handles".

Fat pads can be a nuisance

"If these fat deposits are very disturbing and affect your enjoyment of life, they can be permanently removed with liposuction," explains Prof. Noah, a specialist in plastic and aesthetic surgery. "In no way is liposuction a method for losing weight," he clarifies. The Noah Clinic uses low-risk water-jet assisted liposuction (WAL), which is based on tumescent local anaesthesia, a regional anaesthesia of the skin and fatty tissue. Prof. Noah uses a very fine water jet to gently detach the fatty tissue from the rest of the body. Whether tightening, reduction, enlargement or reconstruction: the reasons for a breast correction are manifold. Prof. Noah fulfils the desire for a beautiful décolleté using the latest medical knowledge and innovative surgical techniques: "The goal is a breast that fits the woman's body and blends harmoniously into the overall picture," explains Prof. Noah. The most frequently requested cosmetic procedure by women is breast augmentation. Augmentation with premium implants or filling with the patient's own fat is possible. It is especially important for the patients to know: At the Noahklinik, only quality implants of the 5th generation (Natrelle® series from Allergan ) with a high level of safety are used. They offer a special strength level of the gel and are particularly tear-resistant.

But missing volume can also be replaced with autologous fat. To do this, excess fat is removed from one part of the body - e.g. the abdomen or thighs - processed and inserted into the breast. This is a good alternative to implants for women who do not want implants or who only want a small enlargement of their breasts. It is also possible to combine both methods. For example, in very slim women, the breast implant is often visible and palpable due to the lack of surrounding fat tissue. "In such cases, we can conceal the transitions very well with autologous fat and create a natural-looking décolleté."

Prof. Noah is currently testing a whole new method that is particularly suitable for lifting large breasts as part of an international study: the "Orbix Breast Support System", also called the "inner bra". In this CE-certified surgical technique, thin silicone sheets are attached to the ribcage with silk straps. This lifts and supports the breasts. This method is not suitable for all women. It is mainly for women who have large breasts and who need firming and internal support. For small breasts, other firming methods are indicated.

Six patients have already been operated on in Kassel as part of this study, and Prof. Noah will surgically lift the breasts of four more patients using this method in June. Other surgical centres for this study are Tel Aviv/Israel, Brussels/Belgium and London/GB. Prof. Noah is the only plastic surgeon in Germany who is testing this surgical method and will evaluate the results together with the other surgeons.

Many women who have had breast implants inserted after cancer surgery or for aesthetic reasons are uncertain and fearful. In Hesse alone, about 70 cases have been reported so far in the past week in which women received silicone cushions that may be dangerous to their health.

In Kassel, our research with all the surgeons in question revealed that no suspicious breast implants from the manufacturers PIP and Rofil were apparently used.

Every day, however, Prof. Dr. Ernst-Magnus Noah, head physician of the Noah Clinic - Clinic for Plastic, Aesthetic and Reconstructive Surgery - receives enquiries from concerned patients. After the disturbing press reports in the past few days, he can understand the excitement. For example, since 6 January, the Federal Institute for Drugs and Medical Devices has been recommending the precautionary removal of all breast implants made by the companies PIP (Poly Implant Prothese) from France and Rofil from the Netherlands.

The plastic surgeon advises concerned women in such cases to first check the implant passport for batch and production info. "Normally, every patient who has received an implant should have an implant card with her medical records," says Noah. However, it also happens that patients have not received an implant card. If in doubt, contact the surgeon or the hospital and ask for the operation report or a written explanation.

This, too, Noah knows, could be difficult for some patients, because it is not uncommon for women to travel abroad, for example to the Czech Republic, to have their breasts enlarged.

Women who wear a PIP or Rofil implant or do not know the manufacturer of their implant should contact a breast centre, a plastic surgeon or their gynaecologist first, recommends Noah, who is active on the board of the Association of German Aesthetic Surgeons, among others. "All the specialist societies involved are pulling in the same direction on this issue," explains the physician.

Women with suspicious implants are advised to have a thorough examination first. With the help of a mammography and ultrasound, perhaps also a magnetic resonance imaging (MRI), one can assess whether an implant may already have cracks. If this is the case, it should be removed as soon as possible, otherwise potentially dangerous implants should be removed as soon as possible.

As soon as the first spring sun shines, many people think anxiously about summer holidays, because swimming trunks and bikinis don't cover up problem zones.

However, a new method - radiofrequency liposuction - offers the possibility of gently getting to grips with the sports- and diet-resistant pads.

Prof. Dr. med. Ernst Magnus Noah, head physician of the clinic for plastic surgery at the Red Cross Hospital Kassel, knows for which problem areas a plastic-aesthetic intervention offers a good opportunity to shape the body: "In men, it is the so-called hip pads, but also too much fat formation in the area of the chest, which is a thorn in the side of many men. For these cases, radiofrequency liposuction is a suitable procedure to improve the body shape."

SKIN IS TIGHTENED

With the new method, excess fat can also be suctioned off over a large area and the skin tightened considerably at the same time," explains Prof. Noah, who is one of the first plastic surgeons to establish the procedure in Germany. "An important advantage of this minimally invasive procedure is that only about three millimetre incisions are necessary under local anaesthesia. This makes this method gentler than others and it is possible to treat several zones in one operation."

In women, it is often fat deposits in the area of the thighs or abdomen that are perceived as disturbing. "Here, too, radiofrequency liposuction is suitable for removing larger amounts of fat under controlled conditions in hospital," says Prof. Noah. Alternatively, depending on the problem, suction technologies such as vibration or water jet technology can also be used.
An important advantage of this minimally invasive procedure is that only about three millimetre incisions are necessary under local anaesthesia. This makes this method gentler than others and it is possible to treat several zones in one operation."

Despite the gentle procedure, patients should still spend a night in hospital to be on the safe side: "Because even though the new methods for treating problem zones sound simple, it is a real operation. So that we can react immediately to irritations, control over a certain period of time is necessary," explains Prof. Noah. "However, as a rule, after major liposuction in several areas, patients can leave the clinic the next day feeling safe and with a tolerable level of pain."

The treatment of the female breast with autologous fat leads to a naturally beautiful result. Human fat cells have a size of approximately 0.1 millimetres and connect with their neighbouring cells via net-like structures to form fatty tissue. There are two types of this tissue with different functions. "The brown tissue is rarely found in the body. It produces energy and thus protects against cold. The white fatty tissue, which I use for the procedure, stores the fat and thus cushions the tissue," Noah explains.

Goal: The skin looks smoother

It is precisely this mechanism that plastic, reconstructive and aesthetic surgery makes use of. "If I inject fat cells under the skin, it fills up from the inside and becomes visibly smoother. This effect is suitable, for example, to even out a deep nasolabial fold. For finer wrinkles around the eyes and mouth, I recommend other filling substances such as hyaluronic acid," the expert explains.

With increasing age, the subcutaneous fat tissue decreases and the elasticity of the skin diminishes. The consequences: Wrinkles and sunken areas of the face. Since autologous fat is a filler substance from the body, the method has significant advantages: There is no risk of rejection or allergy and the effect lasts for a long time. In addition, the injection causes only a small puncture. This keeps post-operative pain to a minimum and the patient is quickly fit for work and social life again.

For autologous fat injection, Noah takes small amounts of fat tissue from the thigh or buttocks and injects it specifically into the area to be treated. Noah also uses the method when it comes to correcting volume loss caused by unsightly scars.

Application area chest

The treatment of the female breast is another, albeit much more complex, field of application for autologous fat transplantation. Not only breast cancer patients benefit from this, but also women with breasts of different sizes or sagging breasts after pregnancy or breastfeeding. "Since the body's own tissue is injected, we achieve a very natural result with this method," says the specialist.

New liposuction methods enable a gentle removal of fat cells. For example, the expert removes fat cells from the tissue using a water jet. These are prepared and then injected into the breast using a special injection technique. "This requires extensive experience and feeling. I therefore advise all those interested to have the procedure performed only by a specialist," Noah concludes. (nh)
